In order to register for an event hosted by Norfolk Minor Hockey Association, this form must be completed in full and included all required attachments.
Completion of this form does not guarantee the submitting team a spot in the event. Registration is on a first come, first served basis and is considered complete once payment has been received.
Registration forms, rosters and travel permits will be accepted by online submission only.
Payment must be received within three weeks following the submission or the registration form in order to guarantee the submitting team a spot in the event.
Event fees are non-refundable (unless the spot can be filled).
